Waiver and Release of Liability

In consideration of the participating player, child or minor named above (“Participant”) being allowed to participate in the Briana Scurry Goalkeeper Clinic (the “Event”), the undersigned, on behalf of themselves and the Participant, agree to the following:

Acknowledgment of Risk
I understand and acknowledge that participation in athletic activities, including soccer, involves inherent risks, including but not limited to injury, illness, permanent disability, or death. I voluntarily assume full responsibility for all risks associated with the Participant’s involvement in this clinic.
Release and Waiver
To the fullest extent permitted by Florida law, I hereby release, waive, discharge, and hold harmless Briana Scurry, Gainbridge, Sporting JAX, Sporting JAX Academy, St. Johns County (Veterans Park), all coaches, staff, volunteers, and any affiliated organizations or sponsors (collectively, the “Released Parties”) from any and all claims, liabilities, demands, or causes of action that may arise from injury, illness, or damages resulting from participation in the Event.
Medical Authorization
In the event of injury or medical emergency, I authorize the clinic organizers and staff to seek medical treatment for the Participant. I agree to be financially responsible for any medical treatment or emergency transportation incurred on behalf of the Participant.
Photo and Media Release
I grant permission for the Participant to be photographed or recorded during the Event. I understand these images may be used for promotional, educational, or informational purposes by the Event organizers or sponsors, without compensation or further approval.
Insurance and Health Acknowledgment
I affirm that the Participant is in good health and physically capable of participating in this soccer clinic. I understand that the Event does not provide health or accident insurance for participants, and that I am responsible for providing such coverage.
Binding Agreement
I certify that I am the legal parent or guardian of the Participant. I have read and fully understand this waiver and release, and I voluntarily sign it with the intent to be legally bound. This agreement shall be governed by the laws of the State of Florida.
